Missed Open Season? Still need to make a change?
[Important notice] The information presented herein does not apply to active federal and postal employees. Active employees must seek information and guidance about changes to their FEHB coverage from their employing agency.
OUTSIDE OF OPEN SEASON CHANGES
Annuitants, survivor annuitants, former spouses, and children or former spouses who are eligible for temporary continuation of coverage and who need to make a change in enrollment outside of the open season, based on a “qualifying life event” (QLE), must use OPM Form 2809.

The QLE determines what type of enrollment change is permitted. The enrollment change must be consistent with the QLE. As an annuitant, you may cancel your enrollment or change to Self Only at any time without experiencing a QLE. Please note that the event of retiring is not a QLE.

An open season enrollment change for annuitants is effective on January 1st. An enrollment change based on a QLE is generally effective the first day of the month after the month in which your request is received.
When such events occur, you must complete OPM Form 2809 which should then be sent via certified mail to the address below.
